Once you’ve filled out the document you wish to submit, please print, sign and return the document by email to customer.experience@bridgewaterbank.ca, by fax to 1.866.841.3537 or by mail to Bridgewater Bank, Suite 150, 926 – 5th Avenue SW, Calgary, AB T2P 0N7.
